The temperature, pulse and respiratory rate are included in measurements referred to as vital signs. Vital signs must be accurately measured because they tell us a great deal about a person’s condition. When a change from normal is seen it means the client should be observed closely. You may also be required to care for clients who require oxygen therapy. This may be a delegation of task (DOT) depending on the facility you work in.
